PASSPORTS

Passport applications must have the following:

Proof of U.S. Citizenship: a certified copy of your birth certificate from state of your birth, or a previous U.S. Passport or Certificate of Naturalization. (Through age 15 – must always have a birth certificate, and if had previous passport or naturalization – must submit also with birth certificate.

Proof of identity, i.e.; valid driver’s license, government ID card, military ID, etc.
One passport photograph measuring 2” x 2” with a light background only.
For minor children from age 16 & 17, evidence of at least one parent’s consent and ID.
Children under the age of 16 (15 and under) must have birth certificate and BOTH parents or legal guardian(s) and the child must appear and present all of the following:

Certified birth certificate, and if had previous passport or naturalization, must submit with birth certificate.
Evidence of child’s relationship to parents or guardian(s), and
Parental identification (valid pictured state or government ID)

Fees: Separate check or money order per application (No Cash or Credit Cards).
        Make payable to U.S. Department of State.
         16 years and older: $110.00 – Good for 10 years
         Under age 16: $80.00 – Good for 5 years

There is a $25.00 fee for the Clerk of Court, payable by check, cash or money order, (No Credit Cards). Applications must be filled out completely; applicant must sign only in presence of clerk – do not sign until you are instructed to do so. Processing time is within 6 weeks.

Passport application Form DS 11 and/or DS-82 can be downloaded from website, www.travel.state.gov or inquiries can be made at 1-877-487-2778 or e-mail @ NPIC@state.gov

Forms Required: DS-82, Application for Passport by Mail, may be used for passport renewal if applicant meets certain qualifications.

DS-11, Application for Passport, is the required form for first-time applicants. It is also the required form for renewals of passports issued more than 15 years ago, or for renewal of passports for minors under 16 years of age.

Renewals:

Renewals can be handled by applicants at home if the following conditions are met using Form- DS-82.

You have been issued a U.S. Passport in your name within the past 15 years, and
Your most recent passport was issued after your 16th birthday, and
You can submit your most recent U.S. Passport with the application.

If you meet the above provisions, complete a Passport Application For DS-82, and you must have the first three items listed:

One passport picture, 2” x 2”
Old Passport
Check or Money Order (No Cash) for $110.00 payable to the

U.S. Department of State
Mail to address on back of application

Expedited Services:

$60.00 per application, PLUS U.S. DEPT OF STATE FEE and postage for the overnight envelope (we supply envelope and stamp fee is included in expedite fee listed below for U.S. DEPT. OF STATE AND CLERK OF COURT). THE CLERK OF COURT FEE IS SEPARATE. PROCESSING TIME WITHIN 3 WEEKS. (TOTAL EXPEDITE FEE FOR A PASSPORT BOOK IS: UNDER age 16-$152.72, 16 and older-$182.72 to U.S. DEPT. OF STATE by check or money order, and the payment to the CLERK OF COURT is $43.30 per person by cash, check or money order). (NO CREDIT CARDS)
